Abstract

Most cancers are still found late, when cure is the exception rather than the rule. Imaging-based screening and single serum markers such as PSA, CA 125, and CEA have helped, but their false-positive rates and weak sensitivity for localized disease leave a wide gap, and blood-based protein profiling offers one plausible way to narrow it. This review examines how computational design is reshaping protein diagnostics, from discovery to the regulatory desk. We describe the analytical platforms used to interrogate the circulating proteome, including data-dependent and data-independent mass spectrometry, proximity extension assays, aptamer arrays, functional protein microarrays, and extracellular vesicle isolation systems, weighing their sensitivity, dynamic range, and throughput against a plasma proteome that spans twelve or more orders of magnitude. We then follow the algorithmic chain that turns thousands of noisy measurements into a compact, locked decision rule: penalized regression, recursive feature elimination, ensemble and neural classifiers, decision fusion, explainable artificial intelligence, and resampling-based validation. Representative panels are synthesized for multi-cancer early detection and for lung, colorectal, esophageal, and liver cancers; several report areas under the curve above 0.95 and encouraging stage I sensitivity, although most derive from case–control designs that probably flatter real-world performance. Finally, we consider what tends to stall translation: pre-analytical variability, high-abundance protein masking, overfitting, incomplete TRIPOD and REMARK reporting, and the demands of FDA 510(k), De Novo, and premarket approval pathways. Computationally designed multi-protein panels look promising, but their clinical value will depend on prospective, population-based validation rather than further gains in retrospective accuracy.

Keywords: early cancer detection; liquid biopsy; plasma proteomics; multi-protein biomarker panel; machine learning; proximity extension assay; mass spectrometry; extracellular vesicles; multi-cancer early detection; regulatory science.
