Bridging the Gap Between Knowledge and Practice: Challenges in Implementing Advanced Cardiac Life Support in Emergency Care

Abstract

Advanced Cardiac Life Support (ACLS) represents a cornerstone of emergency care, yet managing critical cases remains one of the most demanding and high-stakes responsibilities for healthcare professionals. Successful resuscitation hinges not only on physicians’ and nurses’ knowledge but also on their ability to apply guidelines swiftly and accurately under pressure. Despite ongoing updates to ACLS protocols, a persistent gap exists between theoretical understanding and practical execution in real-world clinical settings. This systematic review explores the multifaceted challenges faced by healthcare providers in emergency departments when implementing ACLS. Drawing on a wide range of studies, we examine factors influencing performance, including professional expertise, adherence to protocols, decision-making speed, resource availability, and the role of continuous training. The evidence indicates that proficient application of ACLS significantly enhances patient survival and reduces neurological complications, yet barriers such as inconsistent training, high workload, limited resources, and variable guideline compliance frequently hinder optimal outcomes. Emerging strategies, such as structured continuing education programs, simulation-based training, and integration of clinical decision support tools, demonstrate promise in narrowing the knowledge-practice gap and standardizing resuscitation quality. Recognizing and addressing these challenges is essential not only for improving patient outcomes but also for supporting healthcare professionals in delivering consistent, high-quality critical care. Future policies should prioritize practical training, resource allocation, and guideline reinforcement to ensure equitable and effective ACLS implementation across diverse clinical environments.
