Outcomes of Total Hip Arthroplasty in Ankylosing Spondylitis Patients: A Prospective Study

Abstract
Background: Ankylosing Spondylitis (AS) is a chronic inflammatory disease that primarily affects the axial skeleton, often leading to hip involvement and significant pain. Total Hip Arthroplasty (THA) is an effective surgical option for advanced AS, improving function and quality of life. This study aims to assess the outcomes of THA in AS patients at a tertiary care hospital in Bangladesh. Methods: A prospective observational study was conducted with 50 AS patients who underwent THA between January and December 2022. Preoperative and six-month postoperative assessments were performed using the Harris Hip Score (HHS) and Range of Motion (ROM) measurements. Data on demographics, comorbidities, disease duration, and postoperative complications were collected and analyzed using SPSS 25.0. Results: The mean age of the patients was 38.6 years, with 50% having bilateral hip involvement. The mean HHS significantly improved from 44.2 ± 12.3 preoperatively to 87.2 ± 17.4 postoperatively (p < 0.001). ROM increased from 101.4° ± 40.4° to 202.2° ± 23.5°, with 84% of patients achieving excellent outcomes. No major complications were reported during the study period. Conclusion: THA significantly enhances hip function and quality of life in AS patients, with minimal complications. This demonstrates the safety and effectiveness of THA, especially in resource-limited settings like Bangladesh.
